| Introducing the Health Buddy Program at Bend Memorial Clinic
Bend Memorial Clinic is part of a group that will use a personalized home health monitoring system for patients with chronic conditions, like diabetes, congestive heart failure and chronic obstructive pulmonary disease.
When you participate in the Health Buddy Program, you will get:

A Health Buddy appliance that monitors your health and teaches you about your condition on a daily basis, in the comfort of your own home*.

A care manager with the Bend Memorial Clinic, who assists you with monitoring your health through the Health Buddy Program, and communicates regularly with your doctor.

And you get the peace of mind of knowing that someone is looking out for you.
The Health Buddy Program is being made available through a special Medicare program.

For patients who are unable to use the appliance due to visual, physical or other impairments, the Health Buddy Program can still work for you. Instead of using a Health Buddy appliance in your home, a care manager will monitor your health over the phone. You’ll receive the same coaching, monitoring and assessments over the phone that you would with the Health Buddy appliance.
