| How the Health Buddy Program Works
Medicare has selected approximately 2,500 eligible patients who are already under the care of physicians at the participating medical groups. These patients, who are being treated for congestive heart failure, chronic obstructive pulmonary disease, and/or diabetes, are invited to participate in the Health Buddy program. These health management programs are certified by the National Committee for Quality Assurance (NCQA).
Once enrolled, patients will receive a Health Buddy appliance to use in their homes. The appliance will provide daily coaching, monitoring and education about preventive behaviors that will help keep them healthy. The patients’ responses are automatically sent to a care provider on a secure website.
Doctors and care managers can easily monitor multiple patients simultaneously, using data generated by the Health Buddy appliance to spot problems and intervene early, before patients need to go to the emergency room or be hospitalized.

The Health Buddy Program can still work for patients who are unable to use the appliance due to visual, physical or other impairments. Instead of using a Health Buddy appliance in their home, a care manager will monitor their health over the phone. They’ll receive the same coaching, monitoring and assessments over the phone that they would with the Health Buddy appliance.
