How it WorksBased on a simple home health monitoring technology, the Health Buddy system, this program provides tools and support that make it easier to monitor your patients, help prevent hospitalizations and prioritize your time.
Here's how it works:

Selected Medicare patients will be given a Health Buddy appliance for use in the comfort of their homes. The easy-to-use appliance is programmed with daily dialogues specific to the patients' condition(s). The dialogues include questions about their health and vital signs, as well as coaching, educational information and reminders. |

Once the Health Buddy appliance has collected the information from the patient, the data is available to a Wenatchee Valley Medical Center care manager via a secure website. The patients' responses are risk-stratified and color-coded, so you and the care managers can see at a glance if a patient's condition is showing signs of deterioration.

Care managers will monitor multiple patients simultaneously and alert you to those who need attention – often before the condition requires hospitalization. The website also tracks each patient's trends to provide an overview of an individual's symptom history.
